Position Summary:

Reporting to the Manager of Family Services, the Post Majority Support Worker will be a highly motivated individual who will work with the family to meet their care plan goals and achieve a satisfactory level of wellness and family functioning. They will be required to work with aligning departments such as Protection and Prevention ensuring services are delivered, in accordance with legislation, regulations, policies and accepted practice standards. Further they will support the safety and well-being of youth and young adults aging out of care up to the age of 26. They will assist youth and young adults to connect to internal and external services and resources including connections to family and the community. Post majority care services are aimed to help prioritize safety and support through the assistance of a service provider that can help navigate inclusive and individual well-being journey.

Program goals include:

• Promote strong life skills and holistic well-being. • Promote the social and emotional health; • Help youth and adult youth access and navigate key supports and services; • Increase understanding of the best ways to support youth and their families; • Find gaps in the system and find solutions to the problems families often face.

Duties:

•Participate in planning for transitioning youth/young adults including identifying and maintaining connection to family, extended family and community.
•Contact former and existing youth and young adults in care who meet the post majority criteria to enroll in voluntary services/post majority program.
•Ensure appropriate cultural opening when coordinating meetings within QBOW CFS Inc., for client’s, families, communities, and other collaterals.
•Actively seek guidance from the cultural staff how to incorporate culture into the position or how to work from a cultural perspective.
•Provide wrap-around support that meets the distinct needs and promotes holistic positive outcomes for thriving community youth and young adults.
•Support youth and young adults to be fully independent and identify their own interests to support their well-being for their life-long success.
•Assist youth and young adults to participate in cultural programming.
•Ensure confidentiality and safekeeping of all documents and records.
•Maintain accurate, up-to-date, and concise work files.
•Perform additional related duties in accordance with job responsibilities and department objectives.
•Participate in training, cultural events and other mandatory training as required.
•Performs other duties assigned by the Executive Director/Associate Executive Director.

Required Qualifications, Skills & Abilities:

•Diploma in Indigenous Social Work/Social Work
•3 Years experience working with Indigenous Families in crisis with demonstrated facilitation experience.
•Must have valid Class 5 driver’s license
•Successful candidate will be required to submit a current criminal record check and vulnerable persons check.
•Familiarity with services and support in Saskatchewan, both in the Treaty 4 and Treaty 6 areas.
•Knowledge of First Nation policies and legislations, Saskatchewan Child and Family Services Act, the FSIN Child Welfare and Family Support Act, Policy and Procedures Manual for Family Centered Services (MSS), and other legislation;
•Strong knowledge and awareness of Indigenous culture and history, with emphasis on family violence, child welfare, the impacts of trauma, Indigenous youth/young adults’ engagement, and community outreach.
•Ability to work flexible hours: will include evenings and/or weekends to support program and community needs.
•Able to work efficiently as part of a team as well as independently.
•Good organization, time management and prioritizing skills.
•High level of sound and independent judgement, reasoning and discretion.
•Strong morals and ethics, along with a commitment to confidentiality.
